It's also on JAY SANKEY'S THE ART OF SLEIGHT OF HAND DVD, it says it's based on an old effect by Anneman, so bust out your books. My guess is that it's done like all of Sankey's tricks:

riffle force
double lift
erdnase
mercury card fold
