there are a number of ways..
take a paperclip and straighten it out then put a bend in it so it can fit around your tongue.
there are fake tongues you bite onto and they have a slit in them so the pen can go through
you might be able to use a stealth pen? i doubt it though
you can also use some gum and stick stuff through it.

i use a hanger..remember those arrow thru head gimmicks make it just like that but 2 fit ur tongueand once it goes "THRU" cover the bend with ur finger thats holding ur tongue hope im clear enuff
